If you would like to apply to become an FSG supply partner, you should initially review the details of our assessment criteria and decide if you feel you would be eligible for approval.

If after review of the assessment criteria, you feel you would like to be considered for approval, please click here to download and complete the approval form, along with the Health & Safety Declaration form if your company employs less than 5 people.

Please gather the required supporting information and submit, along with the relevant administration fee, via post to the following;

Your completed application should be accompanied with a cheque, for the appropriate value, made payable to Facilities Services Group.

Please be aware of the following:

• The assessor can take up to 28 working days to complete the initial assessment.
• The assessor will contact you in writing to inform you of their findings. If you are unsuccessful, the assessor will explain the reason for this initial failure and you will have the opportunity to make the required improvements and submit additional evidence or documentation.
• Should the assessor deem the additional work is likely to take more than thirty minutes re assessment work, there is an additional fee payable, as detailed in the table above. You will be notified of this by the assessor as part of your assessment feedback.
• Should you not respond within eight weeks the assessor will close your file and you have to re submit your application.
• There is an annual maintenance charge to maintain compliant status to allow for ratification of annual documentation.
• Becoming an approved FSG Supply chain partner does not guarantee any minimum work volumes but ensures your company is available to our staff and customers to use if they require.

If successful you will receive a formal Facilities Services Group Certificate of Approval, valid for 2 years.
